Hard to say it could be a multitude of things. I don't mean to come across as cruel but there's not enough info in your post to give you a good answer. Maybe you need new players, maybe it's your playcalling or adjustments.

what shell are you usually in on defense cover 3, 4 , 2, 1 ? you have to provide some more information on what your running.
