Seemingly randomly they just wont activate? Sometimes its borderline and youre like "eh, maybe that wasnt quite deep enough by a HAIR for deep KO etc" but not always, just had a couple that def should've been kos and they didn't. Is there a counter to them or does this just randomly happen from time to time?
Only started noticing it very recently fwiw.

Well even if that were true that was in august this only ever happened recently (same with the other poster so far whos noticed it) so not the answer here I'm thinking.
Closest thing to that that would make any realistic sense would be the lazy crooked scum at EA trying to sneak something like that in as a "fix" for part of the crappy game they made and then when they get caught them trying to CLAIM thats the explanation. Lol Wouldnt surprise me actually, sad.
I'm dealing with the same thing man, sometimes KOs just don't do shit lately.

I can only speak for me but if im understanding you correctly I think youre way off. At least as it pertains to this topic that has nothing to do with it. This is about a game mechanic that may literally be not working correctly. Period.
Also I dont think anyone can count on winning because they have the best players. This game is heavily scripted and animation based AND the user has to do everything ourself anyway because the cpu controlled defenders are completely useless. In a roundabout way we're back to the topic, the little bit of help you CAN get from them is if you plan and strategize right to utilize KOs. Sooooo when they suddenly dont seem to work the way they have been, yeah, kinda worth looking at.
They were happening every play for me until this past Thursday

Well, we didn't have defense full of KOs in August either. Most cards didn't have abilities. Now we do, and I'm telling you they do not always work. I probably noticed it a few weeks ago once I got Deep zone and mid zone on the same corner.
I totally agree that it's stupid, especially at this point in the year. It's possible they made KOs worse without saying anything, but I think they just made a shitty game and more people are noticing the cracks that were always there.

I seriously thought it was me, at least initially. I noticed first my Flat Zone KOs weren't lighting up, obviously the sideline and endzone nonsense with KOs bugging out has been a problem all year, but I can't figure out if this is just since a recent patch, or what is going on.
Definitely inconsistent at best. But I'll still take a defense full of KOs blanketing as much of the field as possible over barely working pass rush abilities. Heck, even Inside Stuff feels liek a waste of AP, barely anybody runs. It's a passing league after all.
Thank god those abilities are such a crutch for so many players. Same scrubs who run 6-1 and think it makes them good.

So they fixed the broken KOs from bailing everyone out who can’t user or play defense. isn’t that a good thing? There’s time your wr will take 3 steps and get tapped but a KO and drop the ball still. I think it’s way better with out KO period

No need to insult people who are playing that way, blame EA for making players useless unless they have abilities

User cant be everywhere at once einstein. The defenders should be programmed to be better even WITHOUT kos. But I cant force you to throw to me, and the cpu is easily exploited. Hurry up and rpo etc spam isnt skill either. Nor is endless jukes impressive. Its a game. And a poorly made one. We can only play with what they gave us. So yeah, they should work.
Edit: also offense is super op and too easy so d needs all the help they can get. They need to stop nerfing anything that at least SLOWS down the offense.
