They absolutely shouldn’t if you blitz your user. Setting your user to blitz will take them out of any zone or man coverage assignment making any KO ability pointless.

Yeah this is the way it should work in all regards. For instance, if your User has Mid Zone KO and he's playing man or blitzing when you show play art, the ability should NOT work. I've never tested this as it's common sense, but knowing EA, probably should be tested.
Yes. Plenty of times I don’t think the AI is gonna get there in time, so I will click on, jam the turbo button, and just sprint directly into the receiver.

It shouldn't, no. Your user should have to be in a zone for MZKO to work.
I guess these things should be tested, as there are some variables. For instance (and this will all use MZKO on Sub LB in Nickel for reference), does it rely on the play call (your User's specific assignment)? Like, if you call Cover 3, your man is in a called mid zone, so MZKO should work, yeah? Now say from that C3 you audible him to a blitz, but keep him in coverage. MZKO should NOT work, but what if all that matters is the original play?
Now how bout a man call. Cover 2 Man. Both sub lbers in man, but you audible your user to a hook zone. Does MZKO work? Common sense says yes, but again, if it relies on the playcall, maybe not? And then there's different zones. Like if you audible to a flat zone but play the middle, does MZKO light up, even though your assignment isn't a mid zone?
All kinds of different variables, and this IS EA after all, so who knows what does or doesn't work.

It doesn’t rely on the original play call, it only relies on where the defender is and their assignment when the ball is thrown.
